All participants are expected to behave in a manner that best represents Thomas Road Baptist Church ("Thomas road" or the "Church"). As such, certain items and/or activities will not be tolerated and any person caught in violation of these rules will be sent home at their own expense and will not be given a refund. These items and/or activities include, but are not limited to:
1. Possession or use of alcohol, tobacco products, or illegal/ illicit drugs of any kind
2. Any immoral sexual behavior
3. Verbal or physical abuse
4. Any other rules established by Thomas Road Baptist Church staff or event organizer involved

I, am confident that the adult leaders and sponsors will take appropriate care of y child and every effort will be made for his/her safety. However, I understand that accidents do occur, and in the the event of an emergency, every effort will be made to contact me. In the event of an emergency, I hereby grant permission to a representative of the event organizer, staff member, or adult present, as agent for me, to obtain necessary medical attention in case of sickness or injury to my child as provided by the loco parentis clause of Virginia Code 54.1-2969, to give consent for any X-ray examination; medical, dental, or surgical diagnosis; emergency medical or surgical treatment; and hospital care for my child advised and supervised by licensed to practice under law of the state where the services are rendered, either at a doctors office or in any hospital. I also agree to reimburse any expenses not covered by the Church's insurance.
I, the undersigned parent/guardian of said child, do hereby release, acquit, discharge, and covenant to hold harmless and indemnify Thomas Road
or affiliate for any and all claims, demands, damages, injuries, costs, suits, or causes of action, past, present, or future arising out of any accident or sickness, or treatment thereof, by me or my child while participating in any Thomas Road Baptist Church or affiliate program, event, or activity.
I, the undersigned parent.guardian of said child, do hereby waive and release, indemnify, hold harmless and forever discharge Thomas Road Baptist Church and any affiliate, camp, or Events host organization and their agents, employees, and volunteers from and and all claims, expenses, demands, actions or causes of action, lawsuits, damages, and liabilities, of every kind and nature, whether known or unknown, in law or in equity, that I ever had or may have, arising from, out of, or in any way related to my participation and/or the participation of my child in any events, transportation to and from, and/or any other activities related Thomas Road Baptist Church, provided that this waiver of liability does not apply to any acts of gross negligence, or intentional, willful, or wanton misconduct.
I have read, understood, and fully agree to the above and hereby verify that the above information is true and accurate to the best of my knowledge. Parent/Guardian agrees to provide transportation for child if he/she is found in violation of any of the General Rules.

Check the box if you permit TRMissions and Thomas Road Baptist Church to use any audio, video, written, or pictorial footage of myself taken while on this short-term mission trip for future promotions of TRMissions and TRBC.
I understand that neither TRMissions nor TRBC will use any of this footage for any purposes or organizations outside of marketing for TRMissions and TRBC, nor will they sell or release this material to any outside party.
1. I understand that I am responsible for raising 100% of the funds required for the trip. The money I raise covers, travel costs, food lodging, exit taxes, ministry expense and on the ground transportation. I am responsible for passport, souvenirs, immunizations, food/drink while traveling to and from trip destination.
2. Financial donations made the Thomas Road Baptist Church mission team accounts are not mine- they belong to God and have been given to Thomas Road Baptist Church in order to accomplish the mission and work of the church.
3. If, for some reason, I do not ultimately participate on the mission trip to which I have been accepted, I am responsible for any fees incurred as a result of my cancellation.
4. If I do not raise enough money to pay for my trip, I may not be able to go. Any money raised will be applied to the current trip or materials needed.
5. If I raise an amount of money that exceeds my needs, remaining money will be dispersed to other team members in need or used by Thomas Road Baptist Church for other financial needs associated with the mission.
6. If, for some reason, I do not participate on the mission trip to which I have been accepted, I understand that the monies donated to my trip cannot be refunded to me or the donors due to the non-profit status of Thomas Road Baptist Church. All materials, including airline and other travel tickets, that have been purchased with donated funds cannot be transferred outside of the trip.
7. I understand, should Thomas Road have to cancel a mission trip due to unforeseen circumstances like; insufficient funds, not enough people, dangerous weather conditions, and security reasons. That money cannot be refunded to me or the donors due to the non-profit status of Thomas road Baptist Church. Your money will be set-aside in a fund and available for you to use on another mission trip within the next 12-15 months.
8. In order to comply with the IRS, all checks for support of my trip must be made out to Thomas Road Baptist Church. Donors should write my name and the trip name in the memo line of their check.
9. If inappropriate behavior and/or the breaking of any team covenant or policies causes me to be sent home early from my mission trip as a disciplinary action, none of the money raised will be refunded to me or any donors.
10. If you are a minor and any inappropriate behavior and/or breaking of any team covenant policies occur, you will be sent home at your parent's or guardian's expense.
11. I will return any unused money from my on-field stipend (if given) to my team leader before leaving the airport upon my return.
12. Through personal support-raising for this trip and/or personally paying for the trip, I will have 50% of my funds turned in to Thomas Road Ministries 8 weeks prior to departure, 80% of my funds turned in four weeks prior to departure, and 100% of my funds turned in two weeks prior to departure. I understand that if my personal support for this trip is not raised, I will not be allowed to go on this TRMissions trip.
